Our receptionist will assist you in providing supporting documents like official receipt and insurance form that you may submit to your insurance company for reimbursement. Most of the insurance forms are available on our website under the Insurance Forms menu.

We prefer to book all the appointments in advance in order to provide a better service to all our patients. Please give us a call as soon as possible if you could not make for your scheduled appointment so we could make that space available to another patient in need.

For a new consultation kindly arrive 15 min prior to appointment time as there are several forms that need to be filled before the doctor could see you. All the forms are available on our website under the Patient Forms menu.

We also request you to provide us with vaccinations records, previous medical reports, X-rays and record of current medication.

Our receptionist shall make a reminder call the day before your scheduled appointment.

Walk in patients will be fit into the existing schedule, if possible, or they will be given an appointment date by the receptionist and asked to return later in the day or on the day the schedule allows.
